“Exact” means no rounding anywhere: a fraction stays a fraction and $e$ stays $e$. (A) rounds both coordinates; (B) keeps $x$ exact and then throws $y$ away to a decimal, which is the commonest half-answer. A number this small also rounds to $0$ on a careless display, so the exact form is the only one that survives being read back. The sign convention runs the opposite way to most students’ first guess, because a POSITIVE second derivative means the gradient is increasing, which is what a valley does. Before any calculus happens, turn the words into symbols: a 'rate of change of volume' is $\dfrac{dV}{dt}$, and whatever the question asks you to find is the 'want'. Getting this step right decides which formula you differentiate.

So the formula to differentiate is the one that connects the two letters already in the chain, and the letter to differentiate with respect to is the one the wanted rate is measured against.
