Square $y=x$ to get $y^2=x^2$, integrate to get $\dfrac{x^3}{3}$, substitute the limits, THEN multiply by $\pi$ once at the very end — not at every step along the way. That order is the whole point of this question type.
The formula is $V = \pi\displaystyle\int_a^b y^2\,dx$, so the squaring and the integrating have to happen in one particular order. Do both orders on $y = x + \dfrac{1}{x}$ and say which is the method and what the other one computes.

$\displaystyle\int y^2\,dx$ and $\left(\displaystyle\int y\,dx\right)^{2}$ are not the same object, and nothing in the notation shouts about it. Squaring first is what turns the awkward $x + \dfrac1x$ into three ordinary powers, and the middle term $2 \times x \times \dfrac1x$ collapses to a plain $2$ — the term students most often lose by squaring the two pieces separately. The power rule adds one to the index and divides by the result, and it works for every index except $-1$, where adding one would give a zero denominator — which is precisely the gap $\ln$ was invented to fill. Here the index is $-2$, so the ordinary rule applies and the answer carries a minus sign that comes from dividing by $-1$. (C) forgot the sign and used the old power. “Exact” means the $\pi$ stays a symbol and the fraction stays a fraction. Keeping $\pi$ outside the bracket until the end has a second benefit: the bracket is then plain arithmetic, so a slip in it is visible on its own instead of hiding inside a multiple of $\pi$. On the given interval both brackets are positive and the bars are decoration — but writing them costs nothing and marks you as knowing why they are there.
