This is the single most common slip in parametric differentiation: $\dfrac{dy}{dx}$ is $\dfrac{dy}{dt}$ divided by $\dfrac{dx}{dt}$ — $y$ over $x$, always in that order, never the other way round.
Write the chain rule for a parametric curve, then say which of these two is $\dfrac{dy}{dx}$ and what the other one is:
(A) $\dfrac{dy/dt}{dx/dt}$ (B) $\dfrac{dx/dt}{dy/dt}$
Give a one-second check you can run on any answer that tells you if it has been inverted.

Both fractions are made of the same two derivatives, both are perfectly meaningful, and only one is what you were asked for. The mnemonic is in the notation itself: $\dfrac{dy}{dx}$ has $y$ upstairs, so $\dfrac{dy}{dt}$ goes upstairs. Inverting is the commonest single error in parametric work because the result still looks like a gradient and still gives a plausible tangent. Here the two answers would be $2$ and $\tfrac12$. Three different expressions are all functions of $t$ and all waiting for the same number, so it is easy to feed $t$ into the wrong one. The coordinates always come from the equations the curve was DEFINED by; the gradient always comes from the derivative. Nothing in the working looks wrong if you mix them, and part (c) then builds a line through the wrong point with the wrong slope. The identity is the tool that makes a trigonometric parameter disappear without an inverse function appearing, and it only works on $\cos t$ and $\sin t$ standing alone — which is why both equations are divided by their coefficients first. Squaring and adding then does the whole job in one line. Both rejected points are perfectly good points of the ellipse; they are simply not points of $C$, because the parameter never reaches them.
